Our Health and Wellbeing Team
The Health and Wellbeing Team is here to support the whole person — not just their medical needs.
In the North Devon Coastal PCN, we recognise that health isn’t just about prescriptions and appointments. It’s about what matters to you – whether that’s staying independent, improving mental wellbeing, finding social connection, or getting help during a difficult time.
That’s where our Health and Wellbeing Team comes in.
What the Team Does
Our team is made up social prescribers, all with their own specialities and interests. We also have strong partnerships with local voluntary and community services.
Together, they offer personalised support for a wide range of non-medical needs, helping people take control of their health and wellbeing by making positive changes in all aspects of patients' lives.
How We Can Help
The Health and Wellbeing Team can support with:

Loneliness or social isolation

Long-term health conditions (e.g. diabetes, COPD, heart disease)

Mild to moderate mental health concerns (stress, anxiety, low mood)

Support with finances, housing, or benefits

Carer support and respite options

Access to community groups, volunteering, or physical activity

Healthy lifestyle changes – such as quitting smoking, getting active, or losing weight
You don’t need to be unwell to benefit. Many people are referred simply because they need a bit of extra support navigating life’s challenges.
A Personalised Approach
Our team takes time to listen to what matters most to each person. They’ll work with you to:

Explore your goals, needs, and what’s important to your wellbeing

Connect you to support that suits your circumstances

Follow up regularly and help you stay on track

Coordinate your care if you have multiple services involved

How to Access the Service
Anyone registered with a GP in the North Devon Coastal PCN can be referred into the Health and Wellbeing Team – usually through their GP, nurse, or another member of the practice team.
Some services (like social prescribing or coaching) may also allow self-referral – just ask your practice team for more information.
