North Devon Primary Care network
A Primary Care Network (PCN) is a group of local GP practices working closely together – and with other healthcare professionals, community services, and the voluntary sector – to provide more coordinated and personalised care for patients.
North Devon Coastal PCN includes 3 GP surgeries and two branch sites. We provide quality NHS healthcare services to around 45,000 patients and aim to ensure patients receive proactive, joined-up care tailored to the needs of our diverse population.
We all work together to give our patients to provide excellent quality care to our patients. See below the surgeries that are part of our network:

Why do Primary Care Networks matter?
Living in coastal and rural areas comes with its own unique healthcare challenges. Travel distances, access to specialists, and populations with growing health demands are just a few factors that shape the kind of care our communities need.
North Devon Coastal PCN helps bridge these gaps by:
-
Sharing resources and expertise across practices
-
Delivering specialist services locally, such as pharmacy support, social prescribing, and physiotherapy
-
Supporting patients with complex or long-term conditions through personalised care plans
-
Reducing the need for hospital visits by providing enhanced care in the community
-
Improving access to services, especially for those in isolated areas
