CLEAR Project

We’re committed to supporting the long-term health and well-being of all our patients. As part of our ongoing efforts to prevent and manage Cardiovascular Disease (CVD), we are excited to be taking part in the CLEAR Project. This initiative focuses on identifying and supporting patients who have a Body Mass Index (BMI) over 30, which can increase the risk of developing CVD and other serious health conditions, such as diabetes and hypertension.

Over the next four months, we’ll be reviewing how we can best support patients with a BMI over 30 to reduce their risk of cardiovascular issues. Our aim is to provide personalised care that not only focuses on early detection of CVD, but also on prevention through lifestyle changes, education, and ongoing support. By working closely with our patients, we’ll be able to help them make healthier choices, manage their weight more effectively, and improve their overall heart health.

The CLEAR Project allows us to take a more proactive approach to managing heart health by identifying at-risk individuals earlier and providing them with the tools and guidance they need to make sustainable changes. We understand that factors like stress, mental health, financial strain, and access to healthy food can contribute to poor lifestyle choices, so we’ll also work with our patients to address these underlying causes. Whether it's through lifestyle advice, exercise recommendations, or support with managing weight, our team is dedicated to working alongside our patients to reduce their CVD risk. By providing tailored resources and regular check-ins, we aim to ensure that every patient has the right level of support, taking a holistic approach to improving health and well-being.
